Mechanism of action

Diaflex Rompharm - a drug used in inflammatory and inflammatory-dystrophic diseases of the musculoskeletal system. Diacerein is an anthraquinoline derivative, a diacetylated rhein derivative. Metabolizing to the active metabolite, Rein, inhibits the activity of interleukin-1, which plays an important role in the development of inflammation and degradation of cartilage in osteoarthritis. Inhibits the action of other cytokines (including interleukin-6, TNF alpha). The action develops in 2-4 weeks.

Indications and usage

Primary and secondary osteoarthritis.

Contraindications

Hypersensitivity (including to anthraquinone drugs (laxatives)), pregnancy, lactation, age up to 18 years. Carefully. CKD, irritation of the lower intestines.

Dosage and administration

Diaflex Rompharm is taken orally, after eating, it is necessary to swallow it whole, without chewing, with water, 50 mg 2 times a day (in the morning and in the evening).

Continuously long or in courses (at least 4 months).

Diacerein during the first 2 weeks may cause an acceleration of transit in the intestine, therefore it is recommended to begin treatment with one capsule per day in the evening with food for 4 weeks. Then the dose is increased to 100 mg / day.

Adverse reactions

On the part of the digestive system: diarrhea, nausea, vomiting, abdominal pain, hepatitis.

Allergic reactions: urticaria, fever, bronchospasm, angioedema, anaphylactic shock.

Other: malaise, intense urine staining (depending on pH) from yellow to brown (no clinical significance).

Special notes

The duration of treatment may be long. Before the onset of a therapeutic effect, diacerein can be taken simultaneously with analgesic drugs (including NSAIDs). Periodic monitoring of blood parameters, functional liver and urine samples is necessary. With the deterioration of kidney function, you must reduce the dose of the drug.

In patients with chronic renal failure, with CC less than 30 ml / min, the dose of diacerein should be reduced to 50 mg / day and then titrated according to tolerability and clinical efficacy.

Staining urine can make it difficult to interpret the results of a urine test (for example, glucose content).

Drug Interactions

Antacids reduce the absorption of the drug. When taken simultaneously with drugs that affect the intestinal microflora (including antibiotics), with substances that change the quality of the intestinal contents and the speed of its emptying (fiber), the frequency of intestinal side effects increases.

Drugs containing aluminum hydroxide and / or Magnesium hydroxide, reduce the bioavailability of diacerein.

Simultaneous use with laxatives is not recommended.

Overdosage

Symptoms: diarrhea, weakness.

Treatment: symptomatic. In severe diarrhea, plasma electrolyte control is necessary.
