Mechanism of action
Diflazon - antifungal medication. Fluconazole, a representative of the class of triazole antifungal agents, is a powerful selective inhibitor of sterol synthesis in the cell of fungi. It is active against pathogens of opportunistic mycoses, including caused by Candida spp., Cryptococcus neoformans, Microsporum spp., Trichophyton spp. Fluconazole activity has also been shown in models of endemic mycoses, including infections caused by Blastomyces dermatitidis, Coccidioides immitis and Histoplasma capsulatum.
Indications and usage
Cryptococcosis, including cryptococcal meningitis and other localizations of this infection (including the lungs, skin), both in patients with a normal immune response and in patients with various forms of immunosuppression (including in AIDS patients, in organ transplants ) prevention of cryptococcal infection in patients with AIDS;
generalized candidiasis, including candidemia, disseminated candidiasis and other forms of invasive candidal infections (peritoneal, endocardial, eye, respiratory and urinary tract infections). Treatment can be carried out in patients with malignant tumors, patients of intensive care units and undergoing a course of cytostatic or immunosuppressive therapy, as well as in the presence of other factors,predisposing to the development of candidiasis;
mucosal candidiasis, incl. oral cavity and pharynx (including atrophic candidiasis of the oral cavity associated with the wearing of dentures), esophagus, non-invasive bronchopulmonary candidiasis, candiduria, candidiasis of the skin;
prevention of recurrence of oropharyngeal candidiasis in AIDS patients;
genital candidiasis: vaginal candidiasis (acute and chronic recurrent), prophylactic use in order to reduce the frequency of recurrences of vaginal candidiasis (3 or more episodes per year);
prevention of fungal infections in patients with malignant neoplasms that are prone to such infections as a result of Chemotherapy with cytostatics or radiation therapy; skin mycoses, including mycoses of the feet, body, inguinal region;
onychomycosis; skin candidiasis;
deep endemic mycoses, including coccidioidomycosis, paracoccidioidomycosis, sporotrichosis, and histoplasmosis in patients with normal immunity.
Hypersensitivity to fluconazole, other components of Diflazon or other azole compounds; simultaneous administration of terfenadine (against the background of continuous administration of fluconazole at a dose of 400 mg / day or more), cisapride or astemizole and other drugs that prolong the QT interval and increase the risk of developing severe rhythm disturbances; lactation period; children's age up to 3 years (for this dosage form).
Gastrointestinal: nausea, diarrhea, flatulence, vomiting, abdominal pain, change in taste, rarely - increased activity of liver enzymes and abnormal liver function (jaundice, hyperbilirubinemia, increased activity of ALT, ACT and alkaline phosphatase, hepatitis, hepatocellular necrosis, including with lethal the outcome).
Nervous system: headache, dizziness; rarely convulsions.
From the side of blood-forming organs: rarely - agranulocytosis, neutropenia. Hematological changes (leukopenia and thrombocytopenia) may occur in patients with severe fungal infections.
Cardiovascular: prolongation of the QT interval on the electrocardiogram (ECG), ventricular flicker / flutter.
Allergic reactions: skin rash, erythema multiforme exudative (including Stevens-Johnson syndrome), toxic epidermal necrolysis (Lyell's syndrome), asthma (often with Acetylsalicylic acid intolerance), anaphylactoid reactions (including angioedema, edema, edema, urticaria, pruritus).
Dosage and administration
Diflazon take orally. The daily dose depends on the nature and severity of the fungal infection. The average dose of 200-400 mg 1 time per day.
Treatment can begin in the absence of seeding results or other laboratory tests, but if they are available, an appropriate correction of fungicidal therapy is recommended.
Keep out of reach of children at a temperature not exceeding 25 ° C.
