Mechanism of action
DEVIRS is a unique ribavirin preparation in the form of a cream for the treatment of genital herpes. Today, ribavirin has the widest spectrum of direct antiviral activity against most RNA and DNA viruses. DNA viruses are most sensitive to ribavirin, especially the herpes simplex virus.
Ribavirin triphosphate (RTF) is a potent inhibitor of inosine monophosphate dehydrogenase (IMP-dehydrogenase), viral RNA polymerase, and viral mRNA guanylyltransferase. Inhibition of the latter stops the mRNA capping, which leads to a significant depletion of the intracellular stores of guanosine triphosphate and inhibition of the synthesis of viral RNA and protein.Ribavirin is also inserted into the viral genome, causing lethal mutations, followed by a decrease in the pathogenicity of the virus. Ribavirin inhibits the replication of new virions, which reduces viral load, selectively inhibits the synthesis of viral RNA, without suppressing the synthesis of RNA in normally functioning cells. DNA viruses most susceptible to ribavirin are the herpes simplex virus, adenoviruses, CMV, smallpox viruses, Marek's disease; RNA viruses - influenza A, B viruses, paramyxoviruses (parainfluenza, mumps, peptochemic syndrome, pepticitis); (hemorrhagic fever virus with renal or pulmonary syndrome), oncogenic RNA viruses.
Ribavirin-insensitive DNA viruses: Varicella zoster, pseudo-rabies virus, cowpox; RNA viruses: enteroviruses, rhinoviruses, encephalitis virus of the Semliki forest.
Indications and usage
The cream can be used both as a monotherapy and in combination with other dosage forms of ribavirin, intended for parenteral and oral administration in the following diseases:
1. Infections of the skin and mucous membranes caused by viruses Herpes simplex type 1 and 2, of different localization, including in the genital area.
2. Herpes zoster (as part of complex therapy).
Contraindications:
Hypersensitivity to ribavirin or other components of the drug, pregnancy, lactation, childhood and adolescence (up to 18 years).
Dosage and administration
Externally, apply to the affected skin and mucous membranes (previously washed and dried) in a uniform thin layer, slightly rubbing.
Apply 5 times a day until the complete disappearance of symptoms.
In the absence of improvement within 8 days of the drug use, stop taking it and consult a doctor.
To obtain the best effect, treatment should begin as early as possible when the first symptoms of the disease appear.
In the event that cream Devirs? administered in combination with one of the dosage forms of ribavirin for oral or intravenous administration, the dose of the cream should be reduced.
Do not use for the treatment of ophthalmic herpes, do not apply in the eye area.
The drug should be used with extreme caution in patients with renal insufficiency.
Men and women of childbearing age during treatment and for 7 months after the end of therapy should use effective contraceptives.
