Mechanism of action

Accupro - an antihypertensive drug, an ACE inhibitor.

Indications and usage

Arterial hypertension, chronic heart failure.

When conducting monotherapy hypertension The recommended initial dose of Quinapril in patients not receiving diuretics is 10 mg or 20 mg 1 time / day. Depending on the clinical effect, the dose can be increased to a maintenance dose of 20 mg or 40 mg / day, which is usually prescribed in 1 dose or divided into 2 parts. As a rule, the dose should be changed at intervals of 4 weeks. In most patients, it is possible to achieve adequate blood pressure control during long-term treatment by using the drug 1 time / day. In some patients, the dose of quinapril reached 80 mg / day.

At chronic heart failure quinapril is indicated as an adjunct to diuretics and / or cardiac glycosides. The recommended initial dose is then 5 mg 1 or 2 times / day; After taking the drug, the patient should be monitored to detect arterial hypotension. If tolerability is good, the dose can be increased to an effective one, which is combined with concomitant therapy.

Adverse reactions

Nervous system: dizziness, weakness, headache; rarely, paresthesia, mood and sleep disorders.
Cardiovascular: possible hypotension; rarely - tachycardia.
Gastrointestinal: possible dyspeptic symptoms (including dry mouth, anorexia); rarely - stomatitis, abdominal pain, pancreatitis, cholestatic jaundice.
Metabolism: possible hyperkalemia, hyponatremia; rarely, proteinuria, an increase in the level of urea and creatinine in the blood (mainly in patients with impaired renal function).
Respiratory: possible dry cough, bronchitis, rhinitis.
Hemic and lymphatic: rarely - neutropenia, agranulocytosis, thrombocytopenia, anemia.
Urogenital: possible impaired renal function.
From the reproductive system: rarely - impotence.
Allergic reactions: skin rash, angioedema, and other hypersensitivity reactions are possible.
Dermatologic: rarely - alopecia.
Other: rarely, muscle spasms.

Contraindications

History of angioedema associated with treatment with ACE inhibitors; renal artery stenosis; hemodynamic stenosis of the aortic or mitral valve; hypertrophic cardiomyopathy; primary hyper aldosteronism; pronounced impaired renal function (CC <10 ml / min); hemodialysis; primary disease or impaired liver function; condition after kidney transplantation; pregnancy; lactation; children's age (up to 14 years); hypersensitivity to the drug.
