Antianginal means - nitrates and nitrites

The active substance is isosorbide mononitrate.
Mechanism of action
Antianginal, vasodilator. Reduces preload (due to the expansion of peripheral veins) and afterload (due to a decrease in the total peripheral vascular resistance), reduces myocardial oxygen demand, dilates the coronary arteries and improves coronary blood flow, promotes its redistribution in the ischemic regions, reduces the terminal diastolic volume of the left ventricle and decreases the left ventricular volume tension of its walls. Increases tolerance to an exercise stress of patients with an ischemic heart disease, reduces pressure in a small circle of blood circulation.Well absorbed when taken orally. There is a direct dependence of the concentration in the blood on the dose of the drug.
Indications and usage
Angina pectoris (prophylaxis), post-infarction period, chronic heart failure (combination therapy).
Hypersensitivity, acute myocardial infarction with low filling pressure of the left ventricle, cardiogenic shock, circulatory collapse severe hypotension (blood pressure to less than 90 mm Hg), hypertrophic obstructive cardiomyopathy, constrictive pericarditis, cardiac tamponade, brain haemorrhage, primary pulmonary hypertension, glaucoma, pregnancy and breastfeeding.
"Nitrate" headache; hypotension (including orthostatic), accompanied by reflex tachycardia, lethargy, dizziness, weakness; nausea, vomiting, erythema, methemoglobinemia, allergic reactions.
The effect is enhanced by vasodilators, Calcium antagonists, tricyclic antidepressants, alcohol. Barbiturates accelerate biotransformation and reduce the concentration of isosorbide mononitrate in the blood.
Symptoms: increased severity of adverse reactions. Treatment: monitoring of vital functions, symptomatic therapy.
Dosage and administration
Inside, after eating, squeezed liquid, 10-20mg 1-3 times a day.From 3-5 days a single dose can be increased to 40-80mg (not more).
With care prescribed to patients with glaucoma, with increased intracranial pressure. It should not be used to relieve an attack of angina. It is not recommended to drive a car during the treatment period (attention concentration and reaction rate decrease).
