Mechanism of action
Pectrol is a peripheral vasodilator with a predominant effect on the venous vessels.
Antianginal drug of prolonged action.
Reduces preload (expansion of peripheral veins and reduction of blood flow to the right atrium), reduces the end-diastolic pressure in the left ventricle. As a result, the blood flow in the inner layers of the heart muscle improves, the volume of the left ventricle decreases, the workload on the myocardium decreases, and the oxygen demand in the ischemic myocardial regions decreases.
Systemic dilation of the arteries reduces afterload and, therefore, also reduces the oxygen demand of the myocardium. Large subepicardial vessels, smaller coronary arteries, and also collaterals are expanded, which increases blood flow in the ischemic and non-ischemic regions of the heart muscle.
The drug increases exercise tolerance in patients with coronary artery disease.In heart failure, it helps to relieve the myocardium by reducing preload. Reduces pressure in the pulmonary circulation.
Indications and usage
- Prevention of strokes in patients with coronary artery disease.
- Chronic heart failure (as part of combination therapy).
Contraindications
Absolute contraindications:
- Obstructive hypertrophic cardiomyopathy.
- Congestive heart failure with mitral or severe aortic stenosis.
- Acute stage of myocardial infarction with low filling pressure of the left ventricle.
- Acute cardiovascular failure.
- Tamponade of the heart.
- Severe arterial hypotension.
- Shock.
- Low minute cardiac output due to hypovolemia.
- Increased intracranial pressure.
- Intracerebral hemorrhage.
- Traumatic brain injury.
- angle-closure glaucoma.
- Hypersensitivity to nitrates or other components of the drug.
- Hypoxemia.
- Pulmonary heart.

Pregnancy and Breastfeeding
Adequate and strictly controlled clinical studies of the safety of the drug during pregnancy and lactation has not been conducted.

Pectrol is not intended for the relief of strokes.
Against the background of the use of Pectrol in severe occlusion of the coronary vessels, an increase in angina attacks is possible.
Abrupt withdrawal of the drug can cause deterioration of angina.
Against the background of the use of Pectrol should not drink alcohol.
Application for violations of liver function: Pharmacokinetic parameters do not change significantly in patients with impaired liver function.
Application for impaired renal function: Pharmacokinetic parameters do not change significantly in patients with impaired renal function.
Use in pediatrics: Safety and efficacy of the drug in children have not been established.
Impact on the ability to drive vehicles and control mechanisms: In some patients it is possible to reduce the ability to engage in potentially hazardous activities, especially at the beginning of treatment.
Drug Interactions
With the simultaneous appointment of Calcium channel blockers, antihypertensive drugs, vasodilators, antipsychotics and antidepressants, the hypotensive effect of isosorbide-5-mononitrate may be enhanced.
With the joint use of Pectrol and Sildenafil may develop severe arterial hypotension (this combination is contraindicated).
Simultaneous intake of isosorbide-5-mononitrate and ethanol can cause severe hypotension and collapse.
Overdosage
Symptoms: headache, hypotension, dizziness, tachycardia, shortness of breath; in severe cases, sweating, dry mouth, blurred vision, fainting.
Treatment: if the patient is conscious, it is necessary to induce vomiting. In case of arterial hypotension, the patient should be given a horizontal position with the legs elevated. If necessary, conduct symptomatic therapy. In severe cases, infusion therapy and sympathomimetics are indicated. In severe methemoglobinemia, manifested in the form of dry cyanosis of the lips and extremities and respiration of Kussmaul, it may be necessary to prescribe an antidote (toluidine blue).
