Mechanism of action

Aethoxysklerol - lauromacrogol-400 (polidocanol) damages the vascular endothelium, causing protein denaturation. The degree of damage to the endothelium depends on the concentration and volume of the administered drug.

The pressure bandage, which is applied immediately after administration of Ethoxyscleolol, squeezes the walls of the veins, and thus prevents excessive thrombus formation and recanalization of the initially formed parietal thrombus.

The fibrous scar formed in this way ensures hardening of the veins.

Lauromacrogol-400 also has anesthetic properties.

It not only reduces the local excitability of sensitive receptors, but also interrupts feedback through sensory nerve fibers, which causes the absence of pain during sclerotherapy.

Indications and usage

Sclerosis of varicose veins, telangiectasias and hemorrhoids.

Contraindications

Allergic reactions to laurel macrogol-400 and other components of the drug.
Systemic severe disease in the acute stage.

Dosage and administration

Aethoxysklerol should be administered strictly intravenously, including when telangiectasia!

The drug is administered using very thin needles (as for insulin) and a syringe with a freely moving piston. The venous wall should be pierced in a tangent direction, and the drug should be injected slowly, strictly into the lumen of the vein.

Injections are carried out only in the horizontal or elevated position 30-45º above the horizontal position.

After completion of the injection, the injection site is pressed with a swab and a pressure bandage is applied, and the patient is recommended to walk for 30 minutes without leaving the hospital.

After hardening of telangiectasia, a pressure bandage is applied for 2-3 days, for small veins for hardening for 5-7 days, and for medium and large varicose veins for hardening for 3-5 weeks. In case of severe varicose veins, a longer (over several months) use of elastic bandages or medical stockings of II-III compression classes is recommended.

Adhesive compression bandages are recommended on the hip and in the conical shape of the limb to prevent the dressing from slipping.

The result of sclerotherapy is significantly improved with the proper application of a pressure bandage. These dressings are allowed to remove only with a highly raised limb. Before each rising, even for a short time, the bandage is applied again.

Depending on the prevalence and severity of varicose veins, sclerotherapy can be repeated after 1–2 weeks.

The total daily dose of laurel macrogol-400 should not exceed 2 mg / kg body weight.

Thus, a patient with a body weight of 70 kg can be administered 140 mg of laurel macrogol-400 per day, which are contained in 28 ml of Ethoxisclerolol solution 5 mg / ml, in 14 ml of Aethoxysklerol solution 10 mg / ml or in 4.6 ml of Aethoxysklerol solution 30 mg / ml.

As a rule, maximum doses of Ethoxyscleolol are not applied.

For severe varicose veins, sclerosis should be carried out in several stages. At the first stage, especially for patients who are prone to hypersensitivity reactions, no more than one injection of the drug is given. After 1-2 weeks, taking into account the results of the first injection and the size of the sclerosed area, you can repeat the introduction, not exceeding the maximum dose.

Depending on the diameter of varicose veins, various concentrations of Aethoxysklerol are used.

Aethoxysklerol in the form of a 30 mg / ml solution is used for sclerosis of medium and large varicose veins and hemorrhoids of I and II degrees.
Sclerosis of medium and large varicose veins

For hardening of medium and large varicose veins, a solution of Ethoxiscleolol 30 mg / ml is used, and the procedure is carried out in several stages. At the first stage, no more than 0.5-1.0 ml of Aethoxysklerol is administered once. Depending on the result of the first injection and the size of the sclerosed area, several injections can be carried out subsequently (up to 2 ml each), not exceeding the maximum dose of laurent macrogol-400.

In case of formation of a blood clot, it is removed by notching and extrusion.

Sclerosis of hemorrhoids

During one course of therapy, the total dose of Etoxisclerole 30 mg / ml should not exceed 3 ml of solution.

In each hemorrhoidal site, the drug is injected in a dose not higher than 1.0 ml, strictly under the mucous membrane of the node or higher (cranially). Particular care must be taken in men when sclerotic site, located at 11 o'clock, due to the proximity of the urethra and the prostate gland. The amount of Ethoxyscleolol injected here should not exceed 0.5 ml. Care must be taken when administering the drug in order not to damage the internal anal sphincter and prevent its failure.

Depending on the size of hemorrhoids, sclerotherapy can be repeated in 1-2 weeks.

Drug Interactions

With simultaneous use of Ethoxisclerole with other anesthetics, including for general anesthesia, the effect of these drugs on the cardiovascular system may increase (bradycardia, arterial hypotension).
