Active ingredient and dosage form
Rytmonorm - coated tablets:
1 tablet, film coated, contains propafenone hydrochloride 150 or 300 mg; in the blister of 10 pieces, in a box of 5 blisters.
Rytmonorm - solution for injections:
1 ampoule with 20 ml of solution for injections - 70 mg; in the box 5 pcs.
Mechanism of action
Antiarrhythmic. Rytmonorm blocks sodium channels and is a beta-blocker (applies to IC and Class II), reduces the maximum depolarization rate of phase 0 of the action potential and its amplitude.
Indications and usage
Symptomatic, requiring treatment supraventricular tachycardias, incl. with WPW syndrome, or paroxysmal atrial fibrillation. Severe and life-threatening symptomatic ventricular tachycardias.
Contraindications
Severe heart failure, cardiogenic shock (except arrhythmogenic), severe bradycardia, AV and intraventricular conduction disturbances, sick sinus syndrome, electrolyte disturbances, obstructive pulmonary diseases, hypotension, myasthenia gravis.
Pregnancy and Breastfeeding
During pregnancy, especially in the first trimester, and during lactation, Rytmonorm should be taken only as indicated.
Adverse reactions
Perhaps proarrhythmogenic effect, increased heart failure, orthostatic hypotension, dyspeptic disorders, headache, visual and taste impairment, fatigue.
Dosage and administration
Rytmonorm apply in / in, inside, after food, without chewing, washing down with a small amount of liquid. Doses and treatment regimen are selected individually. During the selection of the dose and for maintenance therapy, the daily dose is 450-600 mg; The maximum daily dose is 900 mg in 3 divided doses. In / injections should be administered slowly over 3-5 minutes. A single dose of Rytmonorm is 1 mg / kg body weight (which, with an average weight of 70 kg, corresponds to 20 ml). If necessary, you can increase the single dose to 2 mg / kg (with a body weight of 70 kg, this corresponds to 40 ml). With prolonged infusion, a daily dose of 560 mg (which corresponds to 160 ml) is sufficient.
