Active ingredient and dosage form

Kornam tablets: 1 pill contains terazosin (in the form of hydrochloride dihydrate) 2 or 5 mg;
in a blister of 10 pcs., in a box of 2 blisters.

Mechanism of action

- peripheral postsynaptic blocker a1-adrenoreceptors. It causes the expansion of arterioles and venules, reduces round focal disease and venous return to the heart, resulting in reduced blood pressure. After taking a single dose of the drug, the hypotensive effect persists for 24 hours.
Blocking a1-adrenergic receptors of the smooth muscles of the prostate gland and bladder neck, the drug helps to normalize urination in patients with benign prostatic hyperplasia.
The drug contributes to the normalization of lipid metabolism: lowers cholesterol, triglycerides and low and very low density lipoproteins in the blood, while increasing the number of high density lipoproteins.
With the systematic use of Kornam, there is regression of left ventricular hypertrophy.

Indications and usage

- benign prostatic hyperplasia;
- arterial hypertension.

Dosage and administration

For benign prostatic hyperplasia The initial dose is 1 mg / day at bedtime. Gradually, the dose is increased to 2-10 mg / day until the optimum effect is achieved.The therapeutic effect is usually marked 2 weeks after the start of treatment. To achieve a lasting effect, the course of treatment should be 4-6 weeks.
With hypertension prescribe the drug in the initial dose of 1 mg / day before bedtime. Gradually increase the dose to achieve a clinical effect. Maintenance dose is 1-10 mg 1 time / day. The maximum daily dose is 20 mg. In the case of a temporary cessation of the drug, treatment is resumed according to the same scheme.

Adverse reactions

Cardiovascular: orthostatic hypotension - the “first dose effect” (it was observed in 1% of patients, mainly receiving diuretics or beta-blockers at the same time); rarely - palpitations, peripheral edema, nasal congestion.
From the side of the central nervous system: asthenia, drowsiness; rarely - a violation of clarity of visual perception.
Hemic and lymphatic: decrease in hematocrit, hemoglobin, leukocytes and concentration of all proteins (clinically insignificant and associated with hemodilution).
Other: rarely nausea.

Contraindications

- pregnancy;
- lactation (breastfeeding);
- childhood;
- Hypersensitivity to the drug.

Pregnancy and breastfeeding

Cornam is contraindicated for use in pregnancy.
If necessary, use Kornam during lactation should decide on the termination of breastfeeding.

Special notes

To prevent the development of orthostatic hypotension, treatment should begin with the appointment of Cornam at a dose of 1 mg / day before bedtime, while the patient should be in the supine position. In the case of a temporary cessation of treatment, resume therapy with the same dose.
The level of prostate specific antigen (PSA) does not change.
Influence on ability to drive motor transport and control mechanisms
Within 12 hours after taking the first dose, after increasing the dose or interrupting therapy, it is not recommended to engage in potentially hazardous activities that require increased attention and speed of psychomotor reactions.

Overdosage

Symptoms: arterial hypotension.
Treatment: put the patient down by lowering the head end of the bed. If necessary, prescribe hypertensive agents. Dialysis is not effective, because terazosin is characterized by a high degree of binding to plasma proteins.

Drug Interactions

With simultaneous use of Kornam and antihypertensive drugs of other groups, potentiation of the hypotensive effect is possible.
No clinically significant interaction with simultaneous application Kornama and NSAIDs, analgesics, antibiotics, anticholinergics / sympathomimetic agents, uricosuric, antihistamines, oral hypolipidemic agents, corticosteroids, antacids, sedatives, tranquilizers, diuretics and beta-blockers.

The drug should be stored at a temperature not higher than 25 ° C.
