Indications and usage
Edema caused by heart failure, liver, kidney and lung diseases.
Contraindications
- Hypersensitivity to Torsemide and sulfonamides, anuria;
- hepatic coma and precomatose state;
- renal failure with increasing azotemia;
- arterial hypotension;
- arrhythmias;
- pregnancy;
- lactation period (no data on the use in this period);
- age up to 18 years (efficacy and safety have not been established).
Carefully: with a predisposition to hyperuricemia, gout, with latent and severe diabetes.
Pregnancy and Breastfeeding
Contraindicated in pregnancy. At the time of treatment should stop breastfeeding.
Pills Diuver take orally after meals, once a day. Therapeutic dose - 5 mg per day. If necessary, increase the dose to 20 mg per day, and in some cases up to 40 mg.
Patients of advanced age do not require special selection of doses.
Adverse reactions
From the blood: in some cases, there may be changes in the blood picture (decrease in the number of red blood cells, leukopenia, thrombocytopenia).
Cardiovascular: in some cases, due to thickening of the blood, circulatory disorders and thromboembolism, reduction of blood pressure occur.
From the digestive tract: various dysfunctions of the digestive tract, loss of appetite, dry mouth; rarely - pancreatitis.
From the kidneys and urinary tract: in patients with urinary tract obstruction may cause urinary retention; sometimes - increased levels of urea and creatinine.
From the side of the central nervous system: headache, dizziness, weakness, drowsiness, confusion, convulsions, paresthesias of the extremities.
Liver: increased activity of liver enzymes may be observed.
Changes in laboratory parameters: hypovolemia, impaired water and electrolyte balance, hypokalemia, increased serum uric acid levels, hyperglycemia and hyperlipidemia.
Allergic reactions: pruritus, rashes and photosensitivity.
Special senses: visual disturbances, tinnitus, deafness.
Torsemide increases the sensitivity of the myocardium to cardiac glycosides during hypokalemia and hypomagnesemia.
When used simultaneously with mineral and glucocorticoids, laxatives can increase excretion of potassium.
Strengthens the action of antihypertensive drugs.
Torsemide, especially in high doses, can enhance the nephro- and ototoxic effects of aminoglycoside antibiotics, the toxicity of Cisplatin preparations, the nephrotoxic effects of cephalosporins, and the cardio and neurotoxic effects of lithium preparations.
Torsemide may enhance the action of curare-like muscle relaxants and theophylline.
When combined with large doses of salicylates, the toxic effect of the latter can be enhanced.
Torsemide can reduce the effectiveness of hypoglycemic (antidiabetic) agents.
Consistent or simultaneous administration of Torsemide with ACE inhibitors can lead to a transient decrease in blood pressure. This can be avoided by reducing the initial dose of an ACE inhibitor and reducing the dose of Torsemide (or temporarily canceling it).
NSAIDs and probenecid may decrease the diuretic action of Torsemide.
Kolestiramin can reduce the absorption of Torsemide from the gastrointestinal tract (according to preclinical studies in animals).
Overdosage
Symptoms: there is no typical picture of poisoning; observed - forced urination, accompanied by hypovolemia, electrolyte imbalance, followed by a fall in blood pressure, drowsiness, confusion, collapse. Gastrointestinal disturbances may occur.
Treatment: specific antidote is absent. Symptomatic treatment involves reducing the dose or discontinuation of the drug and the simultaneous replacement of fluid and electrolyte losses.
Special notes
With long-term treatment with Diuver, it is recommended that the electrolyte balance, glucose, uric acid, creatinine and blood lipids be monitored.
Before appointing Diuvera, electrolyte imbalance correction should be carried out.
With caution, take the drug for gout or a tendency to increase the level of uric acid.
In the presence of diabetes mellitus type 1 or 2, it is necessary to control the level of glucose in the blood.
During treatment with Diuver, patients should refrain from driving and activities that require concentration.
