Composition

Furosemide pills: 1 pill contains Furosemide 40 mg;
in the package 10, 20, 30, 40, 50 pcs.

Furosemide injection: 1 ml contains furosemide 10 mg (1 ampoule - 20 mg);
2 ml in an ampoule, 10, 25 pcs. packaged.

Mechanism of action

Furosemide is a potent diuretic with a rapidly developing effect. Violates the reabsorption of sodium ions, chlorine in a thick segment of the ascending part of the loop of Henle. In addition, increases the excretion of ions of potassium, Calcium, Magnesium. The diuretic effect of the drug develops within 5-10 minutes after i / v administration and lasts for 1-2 hours; after ingestion - after 30 minutes, lasts up to 6 hours. With intravenous administration, it causes dilation of peripheral veins, reduces preload, reduces filling pressure of the left ventricle and pressure in the pulmonary artery. Reduces blood pressure.

Indications and usage

- edematous syndrome of various genesis, including in chronic heart failure stage IIB-III, liver cirrhosis (portal hypertension syndrome), nephrotic syndrome;
- pulmonary edema;
- post-traumatic brain edema;
- some forms of hypertensive crisis;
- eclampsia;
- premenstrual tension syndrome;
- the program of forced diuresis in case of poisoning with barbiturates.

Contraindications

- renal failure with anuria;
- hepatic coma and precoma;
- violations of water-salt metabolism: hypokalemia, hyponatremia, hypovolemia (including hypotension) or dehydration;
- I and II trimesters of pregnancy;
- hypersensitivity to sulfonamides.

Dosage and administration

Set individually, depending on the evidence, the clinical situation, the patient's age. During treatment, the dosage regimen is adjusted depending on the size of the diuretic response and the dynamics of the patient's condition.
Inside for adults prescribed in the initial dose of 40-80 mg / day, then, if necessary, gradually increase the dose and with caution. For children a single dose of the drug is 1-2 mg / kg; the maximum daily dose is 6 mg / kg.
In / in (struino) or in / m adult drug is administered 20-40 mg 1 time / day, in some cases - 2 times / day.

Adverse reactions

From the side of water-electrolyte balance and acid-base balance: with long-term use or appointment in high doses - hypovolemia, dehydration, hypokalemia, hyponatremia, hypochloremia, alkalosis; in some cases, hypocalcemia.
Cardiovascular: arterial hypotension, cardiac arrhythmias.
From the side of the central nervous system: dizziness, muscle weakness, cramps; in some cases, when using the drug in high doses in patients with severe impaired renal excretory function - hearing, vision, paresthesia.
Gastrointestinal: dry mouth; rarely - nausea, vomiting, diarrhea.
Metabolism: transient hyperuricemia (with exacerbation of gout), increased levels of urea and creatinine; in some cases - hyperglycemia.
Dermatologic: dermatitis.
Other: in some cases - anaphylactic shock, changes in the pattern of peripheral blood.

With caution, the drug is prescribed for severe cardiovascular insufficiency, with prolonged therapy with cardiac glycosides, as well as in elderly patients with severe atherosclerosis. Prior to treatment, serious disturbances in water and electrolyte balance should be compensated.
Control of laboratory parameters
During treatment, the level of electrolytes, carbonates, urea should be regularly monitored (especially when using doses higher than 80 mg).
Influence on ability to drive motor transport and control mechanisms

Drug Interactions

With the simultaneous use of furosemide with cardiac glycosides increases the risk of glycoside intoxication, and when combined with the use of GCS - the risk of hypokalemia.
With simultaneous use of furosemide potentiates the action of muscle relaxants.
With simultaneous use of furosemide with aminoglycosides or cephalosporins and Cisplatin, their plasma concentrations may increase, which may lead to the development of nephro and ototoxic effects.
With simultaneous use of NSAIDs can reduce the diuretic effect of furosemide.
With simultaneous use of furosemide and antihypertensive drugs hypotensive effect is potentiated.
With simultaneous use with furosemide, the action of hypoglycemic drugs and pressor amines can be weakened.
