Active ingredient and dosage form

Dicynon pills:
each pill contains 250 mg of etamzilat.
100 pieces per pack.

Dicynon injection:
each vial (2 ml) contains 250 mg of etamzilat.
in 2 ml ampoules of 10 or 50 pieces per pack.

Mechanism of action

is a hemostatic, antihemorrhagic and angioprotective substance, normalizes the permeability of the vascular wall, improves microcirculation. Stimulates the formation of platelets and their release from the bone marrow. The hemostatic effect is due to the activation of the formation of thromboplastin at the site of damage to small vessels and the decrease in the formation of prostacyclin PgI2 in the vascular endothelium, Dicynon promotes adhesion and aggregation of platelets, which ultimately leads to stopping or reducing bleeding. Increases the rate of formation of the primary blood clot and enhances its retraction, practically does not affect the concentration of fibrinogen and prothrombin time. Doses of more than 2-10 mg / kg do not lead to greater severity of the effect. With repeated injections, thrombosis is enhanced. The angioprotective effect of Ditsinona is associated with its effect on the endothelium of capillaries and the main substances of the interendothelial spaces.Possessing antihyaluronidase activity and stabilizing Ascorbic acid, prevents degradation and promotes the formation of large molecular weight mucopolysaccharides in the capillary wall, increases the resistance of capillaries, reduces their fragility, and normalizes permeability during pathological processes. Reduces the release of fluid and diapedes blood cells from the vascular bed, improves microcirculation. This angioprotective effect is manifested in the treatment of various diseases associated with primary or secondary disorders of the microcirculation process. Does not have hypercoagulable properties, does not contribute to thrombosis, does not have a vasoconstrictor action. Restores the diseased bleeding time. It does not affect the normal parameters of the hemostasis system. Dicynone practically does not affect the composition of peripheral blood, its proteins and lipoproteins. Slightly increases the content of fibrinogen. Erythrocyte sedimentation rate may decrease slightly. In addition, it normalizes or reduces the pathologically increased permeability and capillary fragility.

Indications and usage

Pills:
Treatment of internal capillary bleeding of various etiologies, especially if the bleeding is caused by damage to the endothelium:
- prevention and treatment of bleeding during and after surgical operations on all well vascularized tissues in otolaryngology, gynecology, obstetrics, urology, dentistry, ophthalmology and plastic surgery;
- prevention and treatment of capillary bleeding of various etiologies and localization: hematuria, metrorrhagia, primary metrorrhagia, menorrhagia in women with intrauterine contraceptives, nasal bleeding, bleeding gums;
- prevention and treatment of diabetic microangiopathy (hemorrhagic diabetic retinopathy, recurrent hemorrhages in the retina, hemophthalmos).
Injection:
Treatment of internal capillary bleeding of various etiologies, especially if the bleeding is caused by damage to the endothelium:
- prevention and treatment of bleeding during and after surgical operations on all well vascularized tissues in otolaryngology, gynecology, obstetrics, urology, dentistry, ophthalmology and plastic surgery;
- prevention and treatment of capillary bleeding of various etiologies and localization: hematuria, metrorrhagia, primary metrorrhagia, menorrhagia in women with intrauterine contraceptives, nasal bleeding, bleeding gums;
- in neonatology: prevention of periventricular bleeding in newborns.

Contraindications
Hypersensitivity to any of the ingredients, thrombosis, thromboembolism, acute porphyria, hemoblastosis.

The optimal dose for adults: 10-20 mg Ditsinona per kg of body weight in 3-4 doses. Therefore, both for prevention and treatment, the dose is 1-2 pills or 1-2 ampoules (IM or IV) Ditsinona 3-4 times a day; in severe cases, 3 pills or 3 ampoules 3-4 times a day.

Recommended dosage for children:10-15 mg Ditsinona per kg of body weight per day, divided into 3-4 doses.

Tablets should be taken with or after meals.

Prophylactically, 1-2 ampoules of Dicinone should be administered intravenously or intramuscularly one hour before surgery, or 2-3 pills for 3 hours. If necessary, you should enter 1-2 ampoules intravenously during the operation. If there is a risk of bleeding, after the operation, prophylactically introduce 2 to 4 ampoules or give 2 to 8 pills evenly within 24 hours. Emergency should be immediately introduced 1-2 ampoules intravenously or intramuscularly, and then every 4-6 hours in 1 ampoule (or 2 tablets).

As a hemostatic and antihemorrhagic agent, Dicynon is used as long as the risk of bleeding persists: as an angioprotector, it is used until the risk of microcirculation disorders disappears (usually several months). A sterile swab is impregnated with Dicynon and applied to the wound.

Etamzilat cannot be used as the only remedy if the patient has bleeding caused by anticoagulants.

Application Ditsinona not recommended in patients with a history of thrombosis or embolism; in patients with atrial fibrillation of auricular etiology and in patients with exertional angina.

Etamzilat can not be mixed in the same syringe with other drugs, but it can be prescribed in parallel with any therapeutic agents, add to the usual infusion solutions.

Adverse reactions

When taking the drug, heartburn, a feeling of heaviness in the epigastric region, headache, dizziness, facial hyperemia, a slight transient decrease in systolic pressure, paresthesia of the lower extremities appear.
