Mechanism of action

Tizanil inhibits the release of excitatory amino acids that stimulate receptors for N-methyl-D-aspartate (NMDA receptors). As a result, at the level of the intermediate neurons of the spinal cord, polysynaptic transmission of excitation is suppressed. Since this mechanism is responsible for excessive muscle tone, when it is suppressed, muscle tone decreases. In addition to the muscle relaxant effect, tizanidine also has a moderate central analgesic effect.

Indications and usage

Painful muscle spasms in diseases of the spinal column (including osteochondrosis, spondylosis, syringomyelia, hemiplegia, cervical and lumbar syndromes), after surgical operations for herniated disc or osteoarthritis of the intervertebral disc, hip pain, spasticity and pain caused by neurological diseases: distracted spinal disc or osteoarthritis of the hip; diseases of the spinal cord, cerebral blood flow disorders, stroke, head injuries, cerebral palsy, convulsions of central origin.

Contraindications

Hypersensitivity, severe insufficiency of the liver and kidneys, pregnancy, breast-feeding (at the time of treatment is stopped), children age. Side effects. Drowsiness, fatigue, dizziness or vertigo, muscle weakness, anxiety, sleep disorders, hallucinations, a slight decrease in blood pressure (with small doses) or hypertension, bradycardia, dry mouth, nausea, gastrointestinal disorders, a temporary increase in serum transaminase activity.

Dosage and administration

With painful muscle spasm: 2-4 mg three times / day. In severe cases, before bedtime additionally take 2-4 mg.
With spasticity of skeletal muscles: The initial daily dose should not exceed 6 mg. divided into 3 doses. The dose can be increased gradually by 2-4 mg with an interval of 3-7 days. The optimal therapeutic effect is usually achieved with a daily dose of 12-24 mg divided into 3-4 doses at regular intervals. The maximum daily dose of 36 mg.
In patients with renal failure It is recommended to begin treatment with a dose of 2 mg 1 time / day. Increasing the dose should be carried out gradually, focusing on portability and efficacy. If it is necessary to increase the dose, first increase the dosage, prescribed 1 time per day, after which increase the frequency of administration.

Adverse reactions

Drowsiness, weakness, dizziness, a slight decrease in blood pressure, dry mouth, nausea, gastrointestinal disturbances, a transient increase in the activity of hepatic transaminases.

Special notes

It was reported cases of liver dysfunction associated with tizanidine, however, when using a daily dose of up to 12 mg, this is unlikely. In this regard, it is recommended to monitor functional liver function tests 1 time per month during the first 4 months of treatment, and if serum ALT and AST levels persistently exceed the upper limit of the norm by 3 times or more, use of Tizanil should be stopped.

Simultaneous use with fluvoxamine leads to a 33-fold increase in the area under the concentration-time curve of tizanidine, resulting in a prolonged decrease in blood pressure, drowsiness, weakness and lethargy.

Overdosage

Symptoms: nausea, vomiting, hypotension, dizziness, drowsiness, miosis, anxiety, respiratory failure, coma.
