pharmachologic effect

Olanzapine is an antipsychotic, anti-manic and mood-stabilizing agent with a broad pharmacological profile due to its effect on several receptor systems. In preclinical studies, an affinity for various receptors (Ki <100 nmol / L) was established: 5-HT 2a / 2c, 5-HT3-, p-HT6-serotonin. D1-, D2-. D3-, D4-, D5-dopamine. M1-5 - muscarinic cholinergic receptors, a1-adreno and H1-histamine receptors. In animal studies that evaluated the effect of olanzapine on behavior, the latter showed antagonism towards serotonin, dopamine and m-cholinergic receptors, which is consistent with the profile of the associated receptors.
In vitro, olanzapine showed greater affinity for 5HT2-serotonin than D2-dopamine receptors, and greater 5-HT2-than D2 activity in in vivo models. Electrophysiological studies have shown that olanzapine selectively reduced the activity of mesolimbic dopaminergic neurons, having little effect on the striatal pathways involved in motor functions. Olanzapine reduced conditional avoidance at doses lower than those causing catalepsy (the effectindicative of motor adverse reactions). Unlike some other antipsychotics, olanzapine enhances the response in the "anxiolytic" test.

Indications for use

For adults
Olanzapine is indicated for the treatment of schizophrenia.
Olanzapine is effective in maintaining clinical improvement in the long-term treatment of patients with schizophrenia who responded to initial treatment.
Olanzapine is indicated for the treatment of manic episodes from moderate to severe.
Olanzapine is indicated for the prevention of relapse in patients with bipolar disorder in whom it has proven effective in the treatment of the manic phase.
In combination with Fluoxetine, olanzapine is indicated for the treatment of therapeutically resistant depression in adult patients.
Olanzapine in monotherapy is not indicated for the treatment of therapeutically resistant depression.

Contraindications

Hypersensitivity to any of the components of the drug. Patients at risk of developing a angle-closure glocoma. It is contraindicated in persons under 18 years of age.

Dosage and administration

Schizophrenia: The recommended starting dose of olanzapine is 10 mg 1 time per day.
Manic episode: the initial dose is 15 mg once a day with monotherapy or 10 mg per day in combination therapy.
Prevention of recurrence of bipolar disorder: the recommended initial dose of olanzapine is 10 mg once a day. Patients receiving olanzapine for the treatment of a manic episode should continue therapy with the same dose to prevent relapse.If a new manic, mixed or depressive episode occurs, olanzapine should be continued (if necessary, adjusting the dose), resorting to additional therapy to treat mood disorders in accordance with clinical need.
When treating schizophrenia, a manic episode and preventing the recurrence of bipolar disorder, it is allowed to subsequently adjust the dose (in the range of 5–20 mg once a day) on the basis of individual clinical status. Exceeding the recommended initial dose should be carried out only after a proper clinical reassessment and should, in a flail, occur at intervals not less than 24 hours. Olanzapine can be taken regardless of the meal, since food does not affect absorption. With the abolition of olanzapine dose should be reduced gradually.
