Mechanism of action

Teopek (Theophylline) refers to phosphodiesterase inhibitors. Theophylline increases the accumulation in the tissues of c-AMP, which reduces the contractile activity of smooth muscles. Theophylline dilates the bronchi, blood vessels (mainly vessels of the brain, skin and kidneys). Reduces pulmonary vascular resistance, lowers blood pressure in the pulmonary circulation. Increases renal blood flow. It has a moderate diuretic effect. Inhibits platelet aggregation. It has a stimulating effect on the respiratory center, increases the frequency and strength of heart contractions.

Slow release of the active substance from the pill ensures the achievement of therapeutic levels of theophylline in the blood 3-5 hours after taking Teopek and lasts for 10-12 hours. Two times the drug allows you to get effective concentrations of theophylline in the blood during the day.In connection with the action of the drug, one of the important indicators to use is the prevention of night and pre-morning effects of bronchospasm.

Indications and usage

Treatment and prevention of broncho-obstructive syndrome with:
- Bronchial asthma.
- Chronic obstructive bronchitis.
- Emphysema of the lungs.
- Dermato-respiratory syndrome.

Contraindications

- Hypersensitivity (including to other xanthine derivatives - caffeine, Pentoxifylline, theobromine).
- Acute myocardial infarction.
- Severe arterial hyper - or hypotension.
- Common atherosclerosis.
- Serious cardiac arrhythmias (severe tachyarrhythmias, extrasystoles).
- Hyperfunction of the thyroid gland.
- Peptic ulcer of the stomach and duodenum in the acute stage.

- Epilepsy.
- Increased convulsive activity.

Use with caution when:

Dosage and administration

Teopek used inside after a meal, drinking water. The pill is not crushed, not chewed and not dissolved in water. The daily dose is divided into 2 doses with an interval of 12 hours. The optimal dose is selected individually, depending on the nature of the disease, the age and weight of the patient.

The drug is prescribed 2 times a day: adults and children over 14 years, 300 mg; children from 7 to 12 years old - 200 mg; children from 3 to 7 years old - 100 mg.

On the first day, treatment begins with half the daily dose in 2 doses.

In the absence of adverse reactions, switch to the full daily dose. The duration of the drug depends on the characteristics of the course of the disease and the therapeutic effect. On average, a course of treatment can last from 2 weeks to 2 months.

Adverse reactions

Nervous system: dizziness, headache, insomnia, agitation, anxiety, irritability, tremor.

Cardiovascular: palpitations, tachycardia (including in the fetus when a pregnant woman is taken in the third trimester), arrhythmias, reduction in blood pressure, cardialgia, an increase in the frequency of strokes.

Gastrointestinal: gastralgia, nausea, vomiting, gastroesophageal reflux, heartburn, exacerbation of peptic ulcer, diarrhea, with prolonged use - loss of appetite.

Allergic reactions: skin rash, itching, fever.

Other: chest pain, tachypnea, sensation of hot flashes to the face, albuminuria, hematuria, hypoglycemia, increased diuresis, increased sweating.

Side effects are reduced by reducing the dose of the drug.

Special notes

In severe diseases of the cardiovascular system, liver, viral infections, as well as in elderly patients, the dose should be reduced. The effectiveness of theophylline may be reduced in smokers.

Since the bronchodilator effect of Teopac develops gradually after 3-6 hours after administration, the drug is not prescribed for the relief of emergency conditions.

Use caution when consuming large amounts of caffeine-containing foods or beverages during treatment.

Drug interactions

Theophylline is not used in conjunction with other xanthine derivatives. In combination with phenobarbital, difenin, rifampicin, isoniazid, Carbamazepine, sulfinpyrazone, phenytoin, there is a decrease in theophylline efficacy, which may require an increase in the used doses of the drug.

When used simultaneously with macrolide antibiotics, Lincomycin, Allopurinol, cimetidine, isoprenaline, beta-blockers, oral contraceptives, epoxacin, and during influenza vaccination, the intensity of action may increase, which may require a decrease in theophylline dose.

Theophylline increases the likelihood of side effects of glucocorticosteroids, mineralocorticosteroids (hypernatremia), agents that stimulate the central nervous system (increases neurotoxicity). The drug inhibits the therapeutic effects of lithium carbonate and beta-blockers.Theophylline potentiates the action of diuretics by increasing glomerular filtration and reducing tubular reabsorption. With caution, theophylline is prescribed simultaneously with anticoagulants. Antidiarrheal drugs and enterosorbents reduce theophylline absorption.

Overdosage

Symptoms: loss of appetite, gastralgia, diarrhea, nausea, vomiting (including with blood), Gastrointestinal bleeding, tachypnea, flushing of the skin of the face, tachycardia, ventricular arrhythmias, insomnia, motor agitation, anxiety, photophobia, tremor, convulsions. In severe poisoning, epileptoid seizures, hypoxia, metabolic acidosis, hyperglycemia, hypokalemia, decreased blood pressure, confusion, renal failure with myoglobinuria can develop.
Treatment: removal of the drug, gastric lavage, the appointment of Activated carbon, laxatives of drugs, intestinal lavage with a combination of polyethylene glycol and electrolytes, forced diuresis, hemosorption, plasma sorption, hemodialysis (not effective, peritoneal dialysis is not effective), symptomatic therapy (including metoclopramid and ondansetron - with vomiting). In the event of seizures, maintain airway patency and oxygen therapy. For the relief of seizure - in / in diazepam, 0.1-0.3 mg / kg (but not more than 10 mg). With severe nausea and vomiting - Metoclopramide or ondansetron (IV).
