Tracrium has a muscle relaxant effect.

Indications and usage

 
Relaxation of skeletal muscles for endotracheal intubation during general anesthesia, surgery, mechanical ventilation.

Contraindications

Hypersensitivity.

Pregnancy and Breastfeeding

Perhaps if the expected effect of therapy outweighs the potential risk to the fetus. Tracrium can be used for caesarean section.

Adverse reactions

Seizures (in patients with predisposing comorbidities); skin redness, transient hypotension, bronchospasm (associated with the release of histamine), Anaphylactic reactions.

Dosage and administration

Trakrium introduced in / in. Adults and children over 1 month.
Bolus introduction
The recommended doses for adults are 0.3–0.6 mg / kg (depending on the required duration of the neuromuscular blockade), which ensure adequate myoplegia for 15–35 minutes.
Tracheal intubation can be performed within 90 s after IV injection of Trakrium at a dose of 0.5–0.6 mg / kg.
The duration of a complete neuromuscular blockade can be increased by administering additional doses at a rate of 0.1–0.2 mg / kg, which is not accompanied by cumulation of the effect of the drug. Spontaneous restoration of conductivity after complete neuromuscular blockade is noted after approximately 35 minutes (determined by the restoration of tetanic reduction to 95% of the original). Neuromuscular conductivity can be quickly restored with the introduction of anticholinesterase drugs (neostigmine, etc.) in combination with atropine without the appearance of signs of recurrence.
Infusion administration
After the initial bolus injection of Trakrium at a dose of 0.3–0.6 mg / kg, it can be administered by continuous infusion at a rate of 0.3–0.6 mg / kg / h to maintain neuromuscular blockade during long surgical operations. This route of administration may be recommended for heart and lung operations.
With artificial hypothermia, accompanied by a decrease in body temperature to 25–26 ° C, the rate of inactivation of atracuria decreases, therefore, to maintain complete myoplegia, the infusion rate should be reduced by about 2 times.
