pharmachologic effect

Peripheral vasodilator with a predominant effect on the venous vessels. It has antianginal and hypotensive effects.
The mechanism of action is associated with dilatation of the blood vessels (mainly postcapillary) and large arteries. Dilatation of the vessels of the postcapillary bed leads to venous deposition of blood and a decrease in venous return of blood to the heart. As a result of lowering of the left ventricle filling (preload reduction), myocardial oxygen demand decreases, which increases capillary blood flow, opens the lumen of previously not functioning intramural coronary vessels, as well as improves oxygen supply to the ischemic subendocardial region.
Dilatation of precapillary vessels leads to a decrease in systolic pressure in the aorta and OPSS (afterload reduction). As a result, the cardiac output of the left ventricle and the myocardial oxygen demand are reduced.
Direct dilatation of the coronary vessels and a small decrease in the resistance of the coronary vessels improve myocardial perfusion and supply it with oxygen without developing the stealing syndrome.

Indications and usage

- prevention of angina attacks;
- rehabilitation treatment after myocardial infarction;
- pulmonary hypertension;
- chronic heart failure (as part of combination therapy).

The dose is selected individually, since it depends on the stage of the disease and the individual need for nitrates. pills of 20 mg appoint 1 tab. 2-3 times / day, 40 mg pills - 1 tab. 2 times / day, 60 mg pills - 1-2 pills / day, 120 mg capsules - 1 capsule / day.
The drug should be taken after meals, without chewing and squeezing a small amount of liquid. With the appointment of more than 1 tab. / Day interval between doses should be at least 8 hours (to ensure the full effect of the drug).

Adverse reactions

Cardiovascular: at the beginning of treatment - severe headache (due to dilatation of the venous bed of the cerebral vessels); perhaps - tachycardia, orthostatic hypotension.
From the side of the central nervous system: drowsiness.
From the digestive tract: in some cases - nausea, vomiting.
Dermatologic: in some cases - erythematous rash.

Contraindications

- acute myocardial infarction;
- acute circulatory failure (shock, vascular collapse);
- severe hypotension (systolic blood pressure less than 90 mm Hg).

Pregnancy and breastfeeding

Use of the drug during pregnancy is possible only in the case when the intended benefit to the mother outweighs the potential risk to the fetus.
If necessary, the appointment of the drug during lactation should decide on the termination of breastfeeding.

Special notes

Cardique retard should not be prescribed to relieve angina pectoris.
Severe headache that occurs at the beginning of therapy, in most cases, decreases significantly or completely disappears while continuing to take the drug.
Alcohol should be avoided during treatment with Cardichet.
Influence on ability to drive motor transport and control mechanisms
The question of the possibility of practicing potentially hazardous activities should be resolved only after evaluating the patient's individual response to the drug.

Overdosage

Symptoms: headache, palpitations, dizziness, arterial hypotension, nausea, vomiting.
Treatment: symptomatic.

Drug Interactions

With simultaneous use of Cardict retard with antihypertensive drugs, cyclic antidepressants, MAO inhibitors, it is possible to enhance its hypotensive action.
With simultaneous use Cardiket retard potentiates the action of dihydroergotamine and ethanol.

Storage conditions

The drug should be stored at a temperature not higher than 25 ° C. Shelf life - 5 years.
