Mechanism of action
Captopril - inhibitor of angiotensin-converting enzyme (ACE)
The mechanism of antihypertensive action is associated with competitive inhibition of ACE activity, which leads to a decrease in the rate of conversion of angiotensin I to angiotensin II, which is a powerful vasoconstrictor. As a result of a decrease in the concentration of angiotensin II, a secondary increase in plasma renin activity occurs due to the elimination of negative feedback during renin release and a direct decrease in aldosterone secretion. In addition, captopril appears to have an effect on the kinin-kallikrein system, preventing the breakdown of bradykinin.
Due to its vasodilating action, it decreases the total peripheral vascular resistance (afterload), the jamming pressure in
pulmonary capillaries (preload) and resistance in the pulmonary vessels; increases heart volume and load tolerance. With prolonged use reduces the severity of left ventricular myocardial hypertrophy, prevents the progression of heart failure and slows down the development of left ventricular dilatation.
It lowers the tone of the outgoing arteriole of the glomeruli of the kidneys, thereby improving intraglomerular hemodynamics, and prevents the development of diabetic nephropathy.
Indications and usage
Arterial hypertension (including renovascular), chronic heart failure (as part of combination therapy), left ventricular dysfunction after myocardial infarction in patients in a clinically stable state. Diabetic nephropathy in insulin-dependent diabetes mellitus.
Dosage and administration
Initial dose Captoprila is 6.25 mg or 12.5 mg 3 times a day, if necessary, the dose is increased to 25 mg 3 times a day. The maximum daily dose is 150 mg.
Adults: 25 mg three times a day, then, depending on tolerability, gradually increase to 100 mg three times a day.The standard dosage is 50-100 mg three times a day, and the maximum daily dose is 450 mg. Contraindications
Captopril is contraindicated in patients with hypersensitivity to this drug or to other angiotensin-converting enzyme inhibitors (patients who have developed angiophrotic edema when treated with another ACF inhibitor), gastrointestinal disorder, immunological diseases, pregnancy, breast-feeding.
Adverse reactions

Other: dry cough.
