Mechanism of action

Novynette is a monophasic hormonal contraceptive drug for oral administration (birth control pills) containing a combination of estrogen (ethinyl estradiol) and a gestogen (desogestrel). Inhibits the gonadotropic function of the pituitary gland, reducing the secretion of FSH and LH, inhibits the development of the follicle and interferes with the process of ovulation. The contraceptive effect is enhanced by increasing the viscosity of cervical mucus and inhibition of implantation of a fertilized egg.

Pregnancy and Breastfeeding

Novynette is contraindicated for use during pregnancy.

Novynette is contraindicated for use during lactation (breastfeeding), because it inhibits lactation and affects the quality of milk. In addition, the active substances are excreted in breast milk.

Reception of Novynette should be stopped 3 months before the planned pregnancy. In the event of pregnancy, the drug should be canceled.

In the absence of menstrual bleeding, the continuation of admission is allowed only after the exclusion of pregnancy.

Before using the drug, it is necessary to conduct a general medical and gynecological examination (including breast examination, cytological analysis of a cervical smear) to exclude contraindications and pregnancy.

Do not recommend to appoint Novynette to patients with a family history of thromboembolism at a young age or a bleeding disorder.

With extreme caution, the drug is prescribed to patients with moderate and mild arterial hypertension, impaired renal function, multiple sclerosis, epilepsy, small chorea, intermittent porphyria, latent tetany, bronchial asthma.

When a migraine-like, unusually severe headache occurs or worsens, with acute deterioration of vision, with suspected thrombosis or embolism, with a sharp increase in blood pressure, with the development of jaundice or hepatitis,in the event of a generalized itching, in the event of or an increase in seizures, with prolonged immobilization, the drug should be immediately canceled.

Due to the minimal estrogen component, the risk of acyclic bleeding increases. With the appearance of moderate intermenstrual bleeding, especially in the first 2-3 months, you should continue taking Novynette, since in most cases these bleeding stop spontaneously. If intermenstrual bleeding does not disappear or recur, a medical examination is required to determine their cause.

Novynette requires a very careful implementation of the rules of admission (regular, at the same time of day, without skipping pills). While taking the drug requires medical monitoring every 6 months.

After suffering viral hepatitis, the use of hormonal contraceptives for ingestion is allowed no earlier than after 6 months, provided that the liver function is normalized.

Against the background of the use of the drug, patients with impaired liver function should be examined every 2-3 months.

In smokers, women over 35 years of age while taking estrogen-containing drugs increase the risk of thromboembolism of different localization. In this regard, a woman who wants to continue taking contraceptives should stop smoking.

In the case of vomiting or diarrhea, pills should be continued, but additional contraceptive methods should be used.

It should stop taking the drug for 6 weeks. before the planned operation.

Drug Interactions

With simultaneous application Novynette with inducers of hepatic microsomal enzymes such as rifampicin derivatives, phenobarbital, anticonvulsants (phenytoin, carbamazepine), dihydroergotamine, some tranquilizers, phenylbutazone, Ketoconazole, and also with broad-spectrum antibiotics (tetracycline, chloramphenicol, ampicillin, neomycin) It is possible to reduce the contraceptive effect of Novynette (these combinations are not recommended).

With the simultaneous use of Novynette with indirect anticoagulants, there may be a violation of the action of anticoagulants (you should carefully control the prothrombin time and, if necessary, adjust the dose of anticoagulants).

With the simultaneous use of Novineta with tricyclic antidepressants, maprotiline, beta-blockers may increase bioavailability and therefore the toxicity of these drugs.

With the simultaneous use of Novineta with oral hypoglycemic drugs or insulin, a violation of the control of the state of carbohydrate metabolism is possible (which may require dose adjustment).

With the simultaneous use of Novineta with Bromocriptine, a mutual decrease in efficiency is possible.

With the simultaneous use of Novineta with hepatotoxic drugs, especially with dantrolene, the risk of a hepatotoxic effect is significantly increased, especially in women over 35 years of age.

Overdosage

Symptoms: nausea, bleeding (after accidentally ingesting a large number of tablets).

Treatment: conduct symptomatic therapy. It is recommended to monitor the state of water and electrolyte balance, liver function. There is no specific antidote.
