pharmachologic effect

Dorzopt Plus - a combined drug, has an anti-glaucoma effect. Each of the components reduces intraocular pressure by reducing the secretion of aqueous humor. Dorzolamide - carbonic anhydrase inhibitor; reduces the secretion of intraocular fluid (slowing the formation of bicarbonate, followed by a weakening of the transfer of Na + and water). Does not cause accommodation spasm, miosis, hemeralopia. Timolol is a non-selective beta-adrenoreceptor blocker with a slight internal SMA. When applied topically reduces intraocular pressure by reducing the formation of aqueous humor and a small increase in its outflow.

Indications

Open-angle (incl. Secondary) glaucoma; pseudoexfoliative glaucoma; intraocular hypertension.

Contraindications

Hypersensitivity, bronchial asthma, bronchospasm, bradycardia, AV block II – III stage, CHF II – III stage, cardiogenic shock, CRF (CK less than 30 ml / min).

Dosage and administration

Instill 1 cap in the conjunctival sac 2 times a day.If several local ophthalmic drugs are used in the treatment, the drug should be injected at intervals of 10 minutes.

Side effects

Dorzolamide: burning, paresthesia, itchy eyes, tearing, blurred vision, irritation of the eyelids, superficial acupuncture keratitis, conjunctivitis, myopia, blepharitis; rarely - iridocyclitis. Allergic reactions, bitter taste in the mouth, nausea, headache, asthenia.

During the period of treatment, it is necessary to regularly measure intraocular pressure, monitor the function of tearing, the integrity of the cornea, the visual field - at least 1 time in 6 months.
With long-term therapy, it is necessary to control the concentration of K +, electrolytes in the serum, blood pH, heart rate, blood pressure, and other parameters of cardiac activity. Care must be taken when driving at night, to limit the activities in poor lighting.
The drug can be used by patients using rigid contact lenses made of polymethyl methacrylate. The preservative contained in the preparation can be deposited on soft contact lenses, so the lenses should be removed before instillation and put on no earlier than 15 minutes after it.
In case of an upcoming surgery using general anesthesia, it is necessary to cancel the drug 48 hours before the operation, since timolol enhances the action of muscle relaxants and general anesthetics.

Drug interaction

Antiglaucoma drugs (beta-blockers, pilocarpine, dipivefrin, carbachol) enhance the effect. With simultaneous use of carbonic anhydrase inhibitors and beta-blockers inside possible additive effect on systemic effects.

Overdose

Symptoms: violation of water and electrolyte metabolism, acidosis, disorders of the central nervous system.
Treatment: symptomatic.
Timolol: conjunctival hyperemia, lacrimation, photophobia, corneal epithelium edema, burning and itching in the eyes, eyelid hyperemia, diplopia, blepharitis, keratitis, decreased corneal sensitivity, ptosis. With prolonged use in high doses, systemic side effects are possible.
