Mechanism of action
Selective beta1-blocker without its own SMA. Reduces intraocular pressure, resorptive effect is minimal; does not cause miosis, spasm of accommodation, hemeralopia, the effect of "veil" in front of the eyes (unlike miotics). The onset of the clinical effect is 1-2 hours, the duration of the clinical effect is 24 hours.
Absorption - 100%, bioavailability - 80-90%. Communication with plasma proteins - 50%, the time of onset of peak concentration when administered orally - 2-4 hours. T1 / 2 - 14-22 hours. Excreted by the kidneys (15% unchanged). Permeability through the BBB and placental barrier is low, secretion from breast milk is insignificant.
Indications and usage
Open-angle glaucoma, the state after laser trabeculoplasty, increased intraocular pressure.
Locally. Instill in the conjunctival sac 1 drop 2 times a day; During the first month of treatment is carried out under the control of intraocular pressure.
Adverse reactions
Local: allergic reactions, discomfort in the eyes, lacrimation, skin flushing, itching of the conjunctiva, keratitis, anisocoria, photophobia. Systemic: insomnia, depressive neurosis.
Contraindications
Hypersensitivity, sinus bradycardia, AV block II II-III, heart failure (in the stage of decompensation). With caution - diabetes, thyrotoxicosis, simultaneous intake of oral beta-blockers, Raynaud's syndrome, pheochromocytoma, pregnancy, lactation, child age (efficacy and safety are not defined).
Special notes
When applied topically, side effects characteristic of systemic use may occur. During the period of use of the drug should not wear contact lenses. It does not affect the size of the pupil, with closed angle glaucoma, use only in combination with miotics. When transferring the patient to betaxolol after treatment with several antiglaucoma drugs, the latter are canceled gradually. During pregnancy and lactation is used if the benefit exceeds the risk of side effects. They are used with caution in patients whose activities require increased attention and quick psychomotor reactions.
Drug Interactions
With simultaneous use of eye drops and beta-blockers inside the risk of additive effects from both intraocular pressure, and manifestations of the systemic action of beta-blockers. Drugs that depress catecholamine deposition (reserpine) increase the risk of hypotension and / or bradycardia.
