Active ingredient and dosage form

eye drops 0.25% and 0.5% - colorless, transparent solution.
1 ml of the preparation contains: Timolol maleate USP, equivalent to timolol - 2.5 or 5.0 benzalkonium chloride BP - 0.1, disubstituted sodium phosphate, disubstituted sodium phosphate, water for injection;
on 5 ml or 10 ml in plastic bottles droppers in a cardboard pack.

pharmachologic effect
Okumed - antiglaucoma remedy - beta-blocker.
Timolol is a non-selective beta-adrenoreceptor blocker. It does not possess internal impathomimetic and membrane stabilizing activity. When applied topically in the form of eye drops reduces both normal and increased intraocular pressure by reducing the formation of intraocular fluid. No effect on pupil size and accommodation. The action of the drug is manifested 20 minutes after instillation into the conjunctival cavity. The maximum reduction in intraocular pressure occurs after 1-2 hours and persists for 24 hours.
Pharmacokinetics
When applied topically, timolol quickly penetrates the cornea. After instillation of eye drops, the maximum concentration of timolol in the aqueous humor of the anterior chamber of the eye is reached in 1-2 hours.
In small quantities enters the systemic circulation by absorption through the vessels of the conjunctiva, nasal mucosa and lacrimal tract. The removal of metabolites of timolol is carried out mainly by the kidneys.
In newborns and young children, the maximum concentration significantly exceeds that in the plasma of adults.

Indications and usage

- Increased intraocular pressure (ophthalmic hypertension);
- Octrythoracter glaucoma;
- Aphakic glaucoma and other types of secondary glaucoma;
- as an additional means to reduce intraocular pressure with closed angle glaucoma (in combination with mystic);
- congenital glaucoma (in case of insufficiency of other therapeutic measures).

Contraindications

Increased individual sensitivity to timolol as well as to patients, incl. History of which includes the following diseases:
- bronchial asthma or other severe chronic obstructive respiratory diseases;
- sinus bradycardia;
- atrioventricular block II or III;
- severe heart failure;
- cardiogenic shock;
- allergic reactions with generalized skin rashes;
- severe atrophic rhinitis;
- corneal dystrophy;
- neonatal period.
Carefully prescribed to patients with pulmonary insufficiency, severe cerebrovascular insufficiency, heart failure, diabetes mellitus, hypoglycemia, thyrotoxicosis, myasthenia, as well as the simultaneous appointment of other beta-blockers, a child.

There is not enough experience in the use of the drug during pregnancy, lactation and in children, but it has been established that timolol passes the placental barrier, as well as breast milk. It is possible to use the drug Ocumed for the treatment of children, pregnant women and nursing mothers, if the expected therapeutic effect exceeds the risk of possible side effects.

Dosage and administration

Locally. At the beginning of therapy, instill 1 drop of the drug. Okumed 0.25% or 0.5% in the affected eye 2 times a day.
If the intraocular pressure with normal use is normalized, the dosage should be reduced - one drop per day. Treatment with the drug Ocumed is carried out, as a rule, for a long time. A break or change in dosage is carried out only as prescribed by the attending physician.

Adverse reactions

Local reactions: irritation, conjunctival hyperemia, eyelid skin, burning and itching of the eyes, tearing or decrease tearing, photophobia, corneal epithelial edema, superficial punctate keratopetiya, gipostezii cornea, diplopia, ptosis, dry eyes, short-term disturbance of visual acuity, blepharitis, conjunctivitis; keratitis
When conducting, antiglaucomatous surgery may develop a detachment of the choroid in the postoperative period.
Since the cardiovascular system: heart failure, bradycardia, bradyarrhythmia, lowering blood pressure, collapse, atrioventricular block, heart failure, transient disorders of cerebral circulation.
On the part of the respiratory system: dyspnea, bronchospasm, pulmonary insufficiency.
From the side of the central nervous system: headache, dizziness, weakness, depression, paresthesias.
From the gastrointestinal tract: nausea, diarrhea. Allergic reactions; urticaria, eczema. Rhinitis, sexual dysfunction, alopecia.
In case of side effects, you should stop using the drug and contact your doctor as soon as possible.

Interaction with other drugs
The joint appointment of the drug Ocumed with eye drops containing epinephrine, can cause pupil dilation.
The specific effect of the drug — the reduction in intraocular pressure is enhanced by the simultaneous use of eye drops containing epinephrine and pilocarpine; it is not necessary to dig in two beta adrenoblockers in eyes.
Reducing blood pressure and slowing heart rate can be amplified by the combined use of the drug with slow Calcium channel blockers, reserpine and other beta-blockers.
Concomitant use with insulin or oral antidiabetic agents may result in hypoglycemia.
It is impossible to use simultaneously with ant and psychotic drugs (neuroleptics) and anxiolytic drugs (tranquilizers). During treatment it is not recommended to take ethanol (perhaps a sharp decrease in blood pressure).
Timolol enhances the effect of peripheral sites to the sites; therefore, it is necessary to remove the drug 43 hours before the planned surgical intervention using general anesthesia.
These data may also apply to drugs that were used shortly before.

You should visit the doctor regularly to measure intraocular pressure and cornea examination.
If the patient wears soft contact lenses, he should not use Ocumed 0.25% and 0.5% eye drops, since the preservative may be deposited in soft contact lenses and have an adverse effect on the eye tissue.
It is necessary to remove hard contact lenses before instillation of the drug and put them on again only after 15 minutes.
Immediately after instillation of the drug, it is possible to reduce the clarity of vision and slow down mental reactions, which can reduce the ability to actively participate in traffic, car maintenance or to perform work without support. To an even greater extent, this occurs with simultaneous use of the drug with alcohol.
When transferring patients to treatment with timolol, a correction of refraction after the effects caused by previously applied myotics may be necessary.
In the case of the forthcoming surgery with general anesthesia, it is necessary to cancel the drug for 43 hours.
