Composition

Pills:  1 pill contains methylprednisolone 4 or 16 mg;
in vials of 30 and 100, in the box 1 bottle.

Lyophilized powder for injection: 1 bottle contains - methylprednisolone sodium succinate 250 mg,

Mechanism of action

Synthetic glucocorticoid drug. It has anti-inflammatory, immunosuppressive and anti-allergic effects; affects carbohydrate, protein, lipid metabolism.
It reduces the number of immunocompetent cells in the focus of inflammation and vasodilation, stabilizes lysosomal membranes, and reduces the production of prostaglandins and leukotrienes.
Methylprednisolone has a more pronounced anti-inflammatory effect and a less pronounced mineralocorticoid effect than cortisol.

Indications and usage

 
- collagenoses (including systemic lupus erythematosus, rheumatoid arthritis, seronegative spondyloarthritis, Sjogren's disease, dermatopolimitozit);
- allergic reactions, anaphylactic shock;
- severe bronchial asthma;
- ulcerative colitis;
- glomerulonephritis;
- viral hepatitis;
- eczema and neurodermatitis requiring treatment with glucocorticoids;
- leukemia, lymphoma, myeloma, lung cancer (in combination with cytostatics);
- relief of nausea and vomiting arising on the background of cytostatic therapy.

Contraindications

 
- tuberculosis;
- acute and chronic bacterial and viral infections (without proper chemotherapeutic protection).

Dosage and administration

Set individually in accordance with the severity of the disease.
Inside for adults prescribed in a maintenance dose of 4-12 mg / day. The initial dose is often significantly higher (16-96 mg / day). If necessary, long-term treatment supportive therapy is carried out with the drug in the minimum effective dose.
The drug is recommended to take in the morning every other day.
V / m or / in prescribe the drug at a dose of 100-500 mg. At shocked and at graft rejection reactions doses can be increased to 30 mg / kg for a short period. When IV injection of the drug in high doses, the duration of the infusion should be at least 10 minutes.
For stopping nausea and vomiting caused by taking cytotoxic drugsMetypred is prescribed in a dose of 250 mg 20 minutes before taking and 250 mg 6 hours after taking cytostatics. If necessary, Metypred in this dose can be re-assigned every 6 hours.
For children the dose of Metypred should be reduced, but at the same time it should be at least 25 mg / day.
Terms of preparation of injection solutions
To prepare the solution for the / m and / in injection, add 4 ml of the solvent from the attached ampoule to the contents of the vial. The resulting solution has a concentration of 62.5 mg / ml.
To prepare the solution for IV injection, 100-1000 ml of a 5% dextrose solution, saline solution or a mixture of these should be added to the prepared solution.

Adverse reactions

On the part of the endocrine system: Itsenko-Cushing syndrome, atrophy of the adrenal cortex.
Metabolism: disorders of carbohydrate metabolism, weight gain, osteoporosis.
Gastrointestinal: possible development of ulcers of the stomach and intestines.
From the side of the central nervous system: mental disorders, increased intraocular pressure.
Cardiovascular: possible development of arterial hypertension.
From the immune status: decrease in resistance to infectious diseases.

Pregnancy and breastfeeding

It is necessary to observe special care when carrying out GCS therapy during pregnancy and lactation.
GCS in small quantities are excreted in breast milk. If necessary, use metipreda during lactation should stop breastfeeding for the entire period of therapy.
Newborn babies whose mothers received GCS in high doses during pregnancy should be examined to rule out the possibility of adrenal insufficiency.

Special notes

Due to the weak mineralocorticoid effect, methylprednisolone cannot be used for replacement therapy in Addison's disease.
Caution should be exercised with the simultaneous use of Metypred with oral hypoglycemic drugs, anticoagulants, salicylates, barbiturates and diuretics.
Use in pediatrics
When conducting methylprednisolone injection newborn and especially premature baby It is necessary to use distilled water as a solvent.

Overdosage

Currently, cases of overdose Metypred not reported.

Drug Interactions

With simultaneous use Metypred can reduce the effect of salicylates.
When used jointly, the effect of Metypred may decrease under the influence of phenytoin, barbiturates.
