pharmachologic effect

Prostaglandin E drug1used for erectile dysfunction. Causes vasodilation, inhibition of platelet aggregation, stimulation of the smooth muscles of the intestine and uterus, suppression of the secretion of gastric juice. Depresses alpha1-adrenergic transmission in the tissues of the penis and has a relaxing effect on the smooth muscles of the cavernous bodies, which determines the effectiveness of the drug for erectile dysfunction.

Indications and usage

- treatment of erectile dysfunction of a neurogenic, vascular, psychogenic or mixed etiology;
- carrying out pharmacological tests in a complex of diagnostic tests for erectile dysfunction.

Caverject used by direct intracavernous injection, using needles 27-30 sizes 1/2 inch long.
The dose of the drug is selected for each patient individually by careful titration under the supervision of a physician.
At Erectile dysfunction of neurogenic genesis The recommended initial dose of the drug is 1.25 mcg. With each subsequent injection, the dose of the drug can be increased by 2 times, but not more than 5 μg, until the minimum effective dose is reached, which causes a sufficient erection with a duration of no more than 60 minutes.
At disorders of erection of a vascular, psychogenic or mixed etiology The initial dose of the drug is 2.5 mg. With each subsequent injection, the dose is increased by 2 times, but not more than 5-10 μg, until the minimum effective dose is reached. If there is no response to a specific dose, then the next higher dose may be administered after 1 hour. If there is a response, the interval between doses must be at least 1 day.
As a rule, the dose used in the treatment does not exceed 60 micrograms. The multiplicity of injections - 1 time / day; the drug is administered 3 times a week.
When using the drug in as a diagnostic tool It is recommended to use a single dose of the drug that causes an erection.
The first injections of the drug should be carried out in the doctor's office by specially trained medical personnel. The injection site is usually located on the dorso-lateral part of the proximal third of the penis.The side of the penis to which the injection is carried out should be periodically changed. Avoid contact with visible veins. The injection site should be cleaned with an alcohol swab.
Treatment by self-injection at home can be started only after a detailed briefing and mastering by the patient of the self-injection technique. Treatment begins with a dose selected by the doctor. Careful and continuous monitoring of patients participating in the self-injection program should be carried out. It is recommended to visit the attending physician at least 1 time in 3 months. Dose adjustment should only be done by a doctor.

Adverse reactions

Local reactions: the most common adverse reaction is pain in the penis; sometimes prolonged erection and priapism may occur; rarely fibrosis, swelling of the penis, rash on the penis, balanitis, hemorrhages at the injection site, inflammation, itching and swelling of the injection site, bleeding from the urethra, heat in the penis, numbness, fungal infection, irritation, hypersensitivity, phimosis, pruritus, erythema, venous discharge, painful erection and impaired ejaculation. In case of violation of the technique of the introduction of the drug may cause hematoma at the injection site.
Systemic reactions: rarely - pain in the testicles, swelling of the testes, pain and tension of the scrotum, increased urination, urinary incontinence; fluctuations in blood pressure, supraventricular extrasystoles, peripheral vascular reactions, dizziness, headaches, hyperesthesia, weakness in the buttocks, localized pain (in the buttocks, legs, genitals, abdomen), pain in the pelvic region, lower back; flu-like syndrome.

Contraindications

- diseases predisposing to the occurrence of priapism (sickle cell anemia, myeloma, leukemia);
- Anatomical deformities of the penis (angulation, cavernous fibrosis, Peroni disease);
- Hypersensitivity to the drug.
It should not be treated with the preparation of patients with a penile implant. Caverject should not be used in men who are contraindicated or not recommended sex life.

Special notes

Caverject is not intended for simultaneous use with other drugs for the treatment of erectile dysfunction.
Before the start of therapy, the diagnosis of amenable to the causes of erectile dysfunction should be made.
The patient should be warned that in the course of treatment he should inform the doctor about all cases of erection lasting more than 6 hours.
Use of the drug does not protect against sexually transmitted diseases, including HIV infection.
Kaverdzhekt's solution cannot be mixed with solutions of other drugs.
Caverject should not be used in women or children and is not intended for use in newborns.

Overdosage

In the event of a prolonged erection and / or priapism, sympathomimetic amines are administered intracavernously; aspirate; if necessary, resort to surgical treatment.

Drug Interactions

With simultaneous use Caverject and anticoagulants (heparin, warfarin) increases the likelihood of bleeding.

Storage conditions

When stored in the original container, the diluted solution of the drug retains its physical, chemical and microbiological stability for 48 hours at room temperature, as well as for 7 days when stored in a refrigerator.
