Mechanism of action

DUFALAK has a hyperosmotic laxative effect, stimulates intestinal peristalsis, improves the absorption of phosphates and Ca + salts, promotes the elimination of ammonium ions.
Lactulose is cleaved by the intestinal flora of the colon into low molecular weight organic acids, which leads to a decrease in pH and an increase in osmotic pressure and, consequently, an increase in the volume of intestinal contents. These effects stimulate intestinal motility and affect stool consistency. Constipation disappears and the physiological rhythm of emptying the colon is restored. In hepatic encephalopathy or hepatic (pre) coma, the effect is attributed to the suppression of proteolytic bacteria by increasing the number of acidophilic bacteria (for example, lactobacilli); the conversion of ammonia to the ionic form by acidifying the contents of the colon; bowel emptying due to lower pH in the colon and osmotic effect; as well as the reduction of nitrogen-containing toxic substances by stimulating bacteria that utilize ammonia for bacterial protein synthesis.
Inhibits the growth of Salmonella in the intestine, reduces the period of bacteria excretion.

- Constipation: regulation of the physiological rhythm of emptying the colon;
- Stool softening for medical purposes (hemorrhoids, operations on the colon and in the area of ​​the anus);
- Hepatic encephalopathy: treatment and prevention of hepatic coma or precoma;
- Intestinal dysbacteriosis; enteritis caused by salmonella, shigella, salmonellosis in the stage of bacteriocarrier; putrid dyspepsia syndrome (in young children as a result of acute food poisoning).

Contraindications

- Galactosemia,
- intestinal obstruction,
- Hypersensitivity to the active component or fructose, galactose, lactose.

Pregnancy and Breastfeeding

Lactulose can be used safely in pregnant and nursing mothers.

Dosage and administration

In the treatment of constipation, oral appoint 15-45 ml in the first 2 days; then switch to a maintenance dose of 10-25 ml.
For children aged 7-14 years, the initial dose is 15 ml, supporting - 10 ml.
For children 3-6 years of age, the initial and maintenance doses are the same and amount to 5-10 ml.
For children under 3 years old, the drug is prescribed in a dose of 5 ml.
As a rule, the dose can be reduced after 2 days of administration, depending on the needs of the patient.
The drug should be taken 1 time per day in the morning with meals.
The clinical effect occurs after 2 days.The dose is increased if there is no improvement in the patient’s condition within 2 days of taking the drug.
In the treatment of hepatic coma and Precom Duphalac, 30-50 ml of syrup is prescribed 3 times / day. Then, they switch to an individually selected maintenance dose, at which soft stools are observed maximally 2-3 times / day on the basis that the pH of fecal masses is in the range of 5.0–5.5.
In acute cases, Duphalac can be administered as an enema in a ratio of 300 ml of Duphalac syrup and 700 ml of water.
For softening stools for medical purposes, the drug is used in the same doses as in constipation.

Adverse reactions

Gastrointestinal: meteorism is possible in the first days (as a rule, passes on its own after 2 days) overdose may be abdominal pain and diarrhea (dose adjustment is required).

Metabolism: when used in high doses for a long time in the treatment of hepatic encephalopathy due to concomitant diarrhea, electrolyte imbalance may occur, and, as a result, seizures, nausea, headache, dizziness, arrhythmias, myalgia, fatigue, weakness.

Special notes

The drug should be used with caution in patients with diabetes, especially in the treatment of hepatic encephalopathy.
With prolonged use of the drug (more than 6 months) in high doses should regularly monitor the level of electrolytes in the blood plasma.
In the absence of a therapeutic effect within 2 days or when constipation resumes after treatment, the patient must consult a doctor.
In the appointment of Duphalac patients with gastrocardiac syndrome should gradually increase the dose to avoid flatulence.
When using the drug in patients with lactase deficiency, it is necessary to control sugar intake. It should be borne in mind that 15 ml of syrup contains up to 1.7 g of galactose and up to 1 g of lactose.
It is not recommended to take Duphalac for 2 hours after taking other medicines.
Influence on ability to drive motor transport and control mechanisms
The use of Duphalac does not affect the speed of psychomotor reactions and the ability to drive a car or control machines and mechanisms.

Antibiotics and antacids with simultaneous use with Duphalac reduce its effectiveness.
When simultaneously taking Duphalac with enteric preparations with a pH-dependent release, it should be borne in mind that lactulose lowers the pH of the contents of the intestine and, therefore, alters the release of these preparations.

Overdosage

Symptoms: abdominal pain, diarrhea.
Treatment: discontinuation of the drug.

Storage conditions

The drug should be stored in a dark place, out of the reach of children, at a temperature of from 10 ° to 25 ° C.
