Active substance

  • Lactitola monohydrate 5 g

In a disposable bag (sachet) 5 g of powder. In the package 6 bags.

Indications for use

  • Constipation.
  • Need for stool regulation for medical purposes:
    • Colon cleansing in preparation for endoscopic and X-ray examinations.
    • Surgical interventions on the rectum, anal sphincter and the area adjacent to it.
  • Intestinal dysbiosis.
  • Hepatic encephalopathy
  • Hepatic precoma and coma.
  • Hyperammonemia.

Pharmacokinetics

Lactitol carries out its effect only in the large intestine, where it completely enters after ingestion. It is absorbed in minimal quantities in unchanged form (no more than 0.5-2% of the accepted dose).

Clinical Pharmacology

In the large intestine, lactitol is broken down by the action of the intestinal flora into low molecular weight organic acids, which leads to an increase in osmotic pressure in the large intestine, an increase in the amount of feces, their softening, easier defecation and normalization of the intestine. The laxative effect usually occurs within 24 hours after the use of the drug (the delay is due to the passage of the drug through the gastrointestinal tract). At the beginning of the course of treatment, a laxative effect of the drug may be delayed, the effect may occur on the second or third day of its use.

In patients with hepatic encephalopathy or hepatic coma (or precoma), the effect is achieved by migration of ammonia from the blood into the colon (due to lower pH, increased osmotic pressure in the intestinal lumen), removal of retained ammonium ions and other nitrogen-containing toxic substances from the colon. Lactitol inhibits proteolytic bacteria and increases the number of acidophilic bacteria.

Contraindications

  • Individual intolerance to lactitol.
  • Galactosemia
  • intestinal obstruction.
  • Suspected organic damage to the gastrointestinal tract.
  • Abdominal pain.
  • Rectal bleeding of unknown origin.

Adverse effects

At the beginning of treatment it is possible

  •

Feeling of discomfort in the stomach, flatulence. These phenomena, as a rule, disappear with continued use of the drug, as it adapts to it.

In the case of the use of high doses for a long time in the treatment of hepatic encephalopathy in a patient, due to diarrhea, an electrolyte imbalance may develop.

Interaction

At the same time taking Exportal with enteric pH-dependent release preparations, it should be noted that lactitol lowers the pH of the intestine.

It is not recommended to use Expalat within two hours after taking other medicines.

Overdosage

  • Symptoms: diarrhea, in rare cases vomiting is possible.
  • Treatment: these phenomena are usually eliminated by lowering the dose of the drug.

Dosage and administration

Inside during the meal, mixing with various drinks (water, tea, coffee, juices, etc.) or liquid food.

The use of the drug in children is recommended under the supervision of a physician.

With constipation and dysbacteriosis

  •

    • Children 1-6 years: 2.5-5 g (1 / 2-1 tsp. Powder) per day.
    • Children 6-12 years: 5-10 g (1-2 teaspoons of powder) per day.
    • Children 12-16 years: 1-20 g (2-4 teaspoons of powder) per day.

The drug should be taken once, the entire daily dose at once, at one time.

The drug should cause stool once a day; after the onset of a stable laxative effect within a few days of use of the drug, its dose can be reduced. In some cases, to obtain the desired effect is enough half the recommended daily dose.

The minimum course of therapy, after which you should consult a doctor - 5 months.

When regulating stools for medical purposes

  • 20 g of the drug, diluted in 1 liter of water, are taken three times (at 12.15 and 18 hours) on the eve of the medical procedures.

With hepatic encephalopathy, hepatic precoma and coma, hypermonomiemia

The dose of the drug should be selected individually for each patient, depending on the severity of the disease and the individual response to the drug. The initial daily dose is usually 0.5-0.7 g per 1 kg of body weight. This dose is divided into three single doses.Accepted with meals. Should be selected dose, causing two "soft" chairs per day.

Precautionary measures

Lactitol has a low calorie (2 kcal / g), does not affect the level of glucose in the blood and can be used in patients with diabetes mellitus.

Throughout the course of the drug is preferably increased fluid intake.
