Active ingredient and dosage form

Trichopol pills white with a yellowish tinge, round biconvex, have a dividing risk. pills turn yellow under the influence of light.
1 pill contains Metronidazole 250.0 mg;
Excipients: potato starch 40.8 mg, gelatin 2.6 mg, starch syrup 6.6 mg;

Mechanism of action

Trichopol is an antimicrobial and antiprotozoal drug of a wide spectrum of action, a derivative of 5-nitroimidazole.
The mechanism of action of metronidazole is the biochemical reduction of the 5-nitro group of metronidazole by intracellular transport proteins of anaerobic microorganisms and protozoa. The restored 5-nitro group of metronidazole interacts with the microbial cell DNA, inhibiting the synthesis of their nucleic acids, which leads to the death of microorganisms.
Active against Trichomonas vaginalis, Giardia intestinalis, Entamoeba histolytica, Lamblia intestinalis, and in relation to obligate anaerobes (spore-and non-spore-forming) - Bacteroides spp. (Bacteroides fragilis, Bacteroides distasonis, Bacteroides ovatus, Bacteroides thetaiotaomicron, Bacteroides vulgatus), Fusobacterium spp., Clostridium spp., Peptostreptococcus spp., Eubacterium spp. Sensitive strains.
To metronidazole are resistant aerobic microorganisms and facultative anaerobes.

Indications and usage

- trichomoniasis (including chronic complicated);
- giardiasis;
- amebiasis and amebic liver abscess;
- infections caused by anaerobic bacteria (including peritonitis, abscesses of the abdominal cavity and liver; endometritis, abscesses of the ovaries and fallopian tubes, postoperative complications; pneumonia, empyema of the pleura, abscess of the lung; meningitis, abscess of the brain; skin infections; infections of bones; sepsis; endocarditis);
- prevention of complications in surgical interventions on the colon, appendectomy, gynecological operations (in patients from high-risk groups, in combination with intravenous or intramuscular injection with cefazolin);
- mixed bacterial infections (aerobic and anaerobic), usually in combination with an appropriately selected antibiotic;
- in the treatment of Helicobacter pylori infections in duodenal ulcer or stomach ulcer, in combination with bismuth and an antibiotic (for example, amoxicillin).

Contraindications
- blood diseases;
- leukopenia (including in history);
- incoordination;
- organic lesions of the central nervous system (including epilepsy);
- hepatic impairment (for administration in high doses);
- I trimester of pregnancy;
- lactation (breastfeeding);
- hypersensitivity to metronidazole or other nitroimidazole derivatives.

Dosage and administration
With trichomonadosis treatment is carried out simultaneously with both sexual partners.
Inside, during a meal or after a meal (or squeezed milk), without chewing, 250 mg 3 times a day.

When amebiasis - on 250 mg 3 times a day, within 5-10 days.
Children: 35 - 50 mg / kg of body weight per day, in 3 divided doses, for 10 days.

When imblioze: children from 2 to 5 years - 250 mg / day;
from 6 to 10 years - 375 mg / day;
over 10 years old and adults - 500 mg / day.
The daily dose should be divided into 2 doses. The course of treatment is usually 5-10 days.
If necessary, according to the decision of the doctor, the treatment can be repeated after 4-6 weeks.

For infections caused by anaerobic bacteria
Metronidazole can be used in monotherapy or in combination with other antibacterial drugs.
The average period of treatment should not exceed 7 days.
Adults and children over 12 years old: 1 - 2 pills of 250 mg 3 times a day with or after meals.
Newborns and children up to 12 years: 7.5 mg / kg body weight, 3 times a day, during or after meals.

For the prevention of infections caused by anaerobic bacteria (before surgery on the abdomen, in gynecology and obstetrics)
Adults and children over 12 years old:
 First, 1 g (4 pills of 250 mg) in a single dose, then 1 pill of 250 mg 3 times a day during or after meals.
Children from 5 to 12 years: 125 mg every 8 hours for 2 days.
Newborns and children up to 5 years: 5 mg / kg of body weight every 8 hours for 2 days.

Adverse reactions

Gastrointestinal: nausea, taste changes, metallic taste in the mouth, dry mouth, loss of appetite, cramped abdominal pain, nausea, vomiting, constipation or diarrhea.
Nervous system: headache, irritability, sleep disturbances, dizziness, ataxia, depression, peripheral neuropathy, convulsions, tinnitus, hearing loss, disorientation, fainting.
Hemic and lymphatic: transient leukopenia and thrombocytopenia; described the case of bone marrow aplasia.
Allergic reactions: skin rash, itching, urticaria.
Local reactions:

Special notes

Patients with a history of indications of side effects from the central nervous system and hematopoietic system during treatment with metronidazole, if necessary, re-use should be under close medical supervision.
When prescribing Trichopol, patients with impaired liver function should correct the dosage regimen of the drug due to the possible cumulation of metronidazole in the body.
Precautions should be prescribed the drug to patients prone to the appearance of edema or patients receiving GCS because of the high sodium content in the solution.
In the period of use of Trichopol there is a darker staining of urine.
On the background of drug therapy, it is possible to obtain false results in determining the activity of liver transaminases, LDH, triglyceride levels and glucose in the blood plasma.
Metronidazole can immobilize treponemas, which leads to a false-positive TPI test (Nelson's test).
It is necessary to avoid simultaneous use of Trikhopol and indirect anticoagulants. If necessary, their joint appointment should be carefully monitored prothrombin time and set the appropriate dose of anticoagulant.

Do not use Trichopol simultaneously with astemizole and terfenadine.

In combination with Amoxicillin is not recommended to use the drug in patients younger than 18 years.

In the treatment of Trichomonas vaginitis in women and Trichomonas urethritis in men must refrain from sexual activity. Necessarily simultaneous treatment of sexual partners.
The treatment does not stop during menstruation.
After treatment of trichomoniasis, control tests should be carried out during three regular cycles before and after menstruation.

Influence on ability to drive motor transport and control mechanisms

Sulfonamides enhance the antimicrobial action of metronidazole.

Pharmacokinetic interaction
With the simultaneous use of Trichopol with Cimetidine, the metabolism of metronidazole in the liver can be reduced, which can lead to a delayed removal and an increase in the concentration of metronidazole in the blood plasma.
With the simultaneous use of Trichopol and phenobarbital, the metabolism of metronidazole is accelerated due to the induction of microsomal liver enzymes, resulting in T1/2 and plasma plasma concentrations of metronidazole decrease.
With the simultaneous use of Trichopol and phenytoin, a decrease in clearance and an increase in plasma concentration of phenytoin is observed.
