Mechanism of action

MIDOCALM is a centrally acting muscle relaxant.

It has a membrane stabilizing, local anesthetic effect, inhibits the conduction of pulses in the primary afferent fibers and motor neurons, which leads to the blocking of spinal mono- and polysynaptic reflexes. It is also likely that it again inhibits the release of mediators by inhibiting the entry of Ca 2+ into synapses. In the brainstem, it removes the relief of arousal along the reticulospinal pathway. Enhances peripheral blood flow regardless of the influence of the central nervous system. The weak spasmolytic and adrenoceptor blocking effect of tolperisone plays a role in the development of this effect.

Indications and usage

- Treatment of pathologically increased tone and spasms of striated muscles arising from organic diseases of the central nervous system, including:
- The defeat of the pyramidal paths.
- Multiple sclerosis.
- Stroke.
- Myelopathy.
- Encephalomyelitis.

- Spondylosis.
- Spondyloarthrosis.
- Cervical and lumbar syndromes.
- Arthrosis of large joints.

Contraindications

- Myasthenia.
- Childhood.
- Hypersensitivity to the drug components, incl. to Lidocaine.

Pregnancy and Breastfeeding

Use during pregnancy, especially in the first trimester, and during lactation (breastfeeding) is possible in cases where the expected benefit of therapy for the mother outweighs the potential risk to the fetus or infant.

Special notes

- The impact on the ability to drive vehicles and control mechanisms of Mydocalm-Richter does not increase the risk of negative consequences when driving vehicles and activities that require concentration of attention and high speed of psychomotor reactions.

Drug Interactions

Data on drug interactions that limit the use of Mydocalma-Richter are not available.

Although tolperisone has an effect on the central nervous system, it does not cause a sedative effect and therefore can be used in combination with sedatives, hypnotics and drugs containing ethanol.

Does not change the effect of ethanol on the central nervous system.

Overdosage

- Symptoms: the drug has a high therapeutic index, the literature describes the use of the drug in children orally at a dose of 600 mg without the appearance of serious symptoms of intoxication. In children, when taking the drug orally at a dose of 300-600 mg / day, in some cases, irritability was observed. In experimental animal studies, ataxia, tonic and clonic convulsions, shortness of breath, respiratory paralysis were observed in the study of acute toxicity following the oral administration of the drug in high doses.
- Treatment: symptomatic and maintenance therapy is recommended. There is no specific antidote.
